Digital Lab Miami · Field Assessment

Initial Assessment Form

The following assessment helps the business owner/rep understand pain points and areas of improvement.

Never write down passwords, card numbers or bank details. Record only who holds an account, and whether access was confirmed.

01

Business snapshot

The basics, so we know who you are and how to reach you. Only the name and one way to contact you really matter — leave the rest blank if you would rather.

More detail, if you have time Optional — eleven more sections about how the business runs day to day. Skip it and we will ask what we need.
02

Domain and website

Ownership matters more than appearance. A business that cannot prove it controls its own domain is one lapsed renewal away from losing its address.

03

Email

04

Phone and customer contact

For most small operators the phone is the business. Missed calls are usually the single largest untracked loss.

Google presence and reviews

For a local business this is usually the highest-leverage item on the whole form, and the cheapest to fix.

Record what exists and who can get into it. Abandoned public accounts do active damage and are worth closing.

Leads, customers and follow-up

Ask them to show you rather than describe it. The gap between the described process and the actual one is where the work is.

In the owner's words

This is the part we care about most. Answer in your own words — there is no wrong answer, and one sentence each is plenty.
